Online Application for ArtSeed Fine Arts Intensive Programs 2019

ARTSEED FINE ARTS SPRING INTENSIVE

For students, ages 6 to 19 years, or for younger students after an interview. Individual portfolio and collaborative project-building workshop with teaching artists, guest presenters, and adult volunteers culminating in a fall exhibition. Weekdays, March 25 – 29, 2019, 9 a.m. – 5 p.m.  Locations: 1) ArtSeed gallery and offices, Suites 206 & 210, 1007 General Kennedy Ave., San Francisco, CA 94129. 2) 4301 Geary Blvd at 7th Avenue, San Francisco, CA 94118.

ARTSEED FINE ARTS SUMMER INTENSIVE

Students will have up to three full weeks of learning adventures in the arts! For ages 6-19 years, though younger participants may apply for the second week if maturity and desire are reflected in a letter or a phone interview. Week One: June 10–14, 2019, 9am–5pm for ages 9 to 19. Week Two: June 17–21, 2019, 9am–5pm for ages 6 to 19. Week Three:  July 8–12, 2019, 9am–5pm for ages 6 to 19.  Locations:  1) ArtSeed gallery and offices, Suites 206 & 210, 1007 General Kennedy Ave., San Francisco, CA 94129. 2) 4301 Geary Blvd at 7th Avenue, San Francisco, CA 94118

This online form should be filled out and sent in by parents or guardians (reservations are on a first come first serve basis). We prefer to receive it at least one week before the first day of ArtSeed’s Summer Intensive. Sliding scale scholarships to cover a portion of the weekly cost are available to eligible families.   • Full cost of the program per student/per week is $400.00 (includes snacks, art supplies, & field trips) If the full cost of the program would be a financial hardship to your family, you may be eligible to apply for a full/partial scholarship.

 

II. Parent/Guardian Permission Form

    I give permission to participate in ArtSeed’s Summer Intensive. I understand that you may be going to urban areas in transition that have incidents of gang-related activities & properties that may have been identified as containing environmental hazards.

  • I also give permission for the above-named student to participate in the following activities offered by the ArtSeed Program, through Josefa Vaughan and her associates.

  • If this student does NOT have my permission to leave the ArtSeed Program alone, he/she will be picked up by one of the following designated individuals:

 

III. ArtSeed Policies

    The ArtSeed Program requires that each student’s parents/guardians understand and accept its policies on the following issues. Please read the policies listed below & sign your name below to indicate your understanding & acceptance of these policies

  • I ACKNOWLEDGE THAT THIS GENERAL RELEASE OF LIABILITY OF ARTSEED IS BINDING ON ME PERSONALLY AND ON MY HEIRS, PERSONAL REPRESENTATIVES, SUCCESSORS, AND ASSIGNS. THE UNDERSIGNED HAS READ AND VOLUNTARILY SIGNS THE ARTSEED PARENT/GUARDIAN PERMISSION FORM

  • ArtSeed is a non-profit tax-exempt fine arts / youth leadership organization under Internal Revenue Code 501(c)(3) & Revenue and Tax Code 27301d. Any charitable contribution, made w/o expectation of material benefit, is tax deductible. EIN: 52-2368513.
